How to Know When Your Child Needs Therapy — and What to Do Next

As a parent, you know your child better than anyone. You notice the little things — the way they hold their spoon, the words they do or don't say, how they react when the routine changes. Sometimes those observations come with a quiet worry: Is this normal? Should I be concerned? Am I overreacting?

The short answer is: you're never overreacting. If something feels off, it's always worth exploring. Early intervention is one of the most powerful tools we have in pediatric therapy, and getting started sooner rather than later can make a world of difference.

Signs Your Child May Benefit from Therapy

Every child develops at their own pace. But there are certain patterns that may indicate your child could benefit from professional support. Here are some common signs, broken down by therapy type:

Speech & Language Concerns

  • Your child isn't babbling by 12 months or saying single words by 16 months
  • They have trouble being understood by family members by age 2-3
  • They get frustrated when trying to communicate
  • They aren't putting two words together by age 2
  • They seem to understand less than other children their age

Motor Skill & Movement Concerns

  • Your baby isn't rolling, sitting, or crawling on the expected timeline
  • Your toddler seems clumsier than peers, falls frequently, or avoids physical activities
  • Your child has difficulty with fine motor tasks like holding crayons, using scissors, or buttoning clothes
  • They walk on their toes consistently
  • They seem to have low muscle tone or tire easily

Sensory & Behavioral Concerns

  • Your child is extremely bothered by certain textures, sounds, or lights
  • They have intense meltdowns that seem disproportionate to the situation
  • They struggle with transitions or changes to routine
  • They have difficulty playing with other children
  • They avoid certain foods due to texture (beyond typical picky eating)

"But My Pediatrician Said to Wait..."

This is something we hear often. Many pediatricians take a "wait and see" approach, especially with speech and motor milestones. And while some children do catch up on their own, research consistently shows that early intervention leads to better outcomes.

You do not need a referral to start the process at most pediatric therapy clinics, including ours. You also do not need a formal diagnosis. If your gut is telling you something isn't quite right, that's enough of a reason to schedule a screening.

What Happens During a Screening?

A developmental screening is typically a quick, no-pressure meeting where a therapist observes your child in a play-based setting. They'll look at how your child moves, communicates, and interacts. It's not a test your child can "fail" — it's simply a way for a trained professional to see what's going on and determine if a full evaluation is needed.

What If My Child Does Need Therapy?
